What
Is Hypoglycemia And Why Should I Care?by Anita Flegg
Hypoglycemia literally means Low Blood Sugar and the best understood cases of hypoglycemia occur in diabetics. In diabetes, the blood sugar is maintained through medication and keeping the level steady can be a challenge. Too much insulin, too little food or too much exercise can all trigger hypoglycemia in diabetics.
There are many people experiencing symptoms, though, and they are not diabetic. Increasing numbers of doctors and lay people are coming to believe that hypoglycemia is really quite common in the general population. Many sources estimate that 10-25%—some sources say as many as 40%!—of non-diabetics are dysglycemic, meaning that they are unable to deal properly with the sugar in their diets.
So why should we care?
First, hypoglycemia results in lost hours, lost years and lost productivity because sufferers are sluggish, light-headed, exhausted and often overweight. Second, chronic hypoglycemia causes constant stress on the body, and can lead to Type II diabetes, Insulin Resistance and Metabolic Syndrome.
In the most simplistic terms, the human body runs on sugar. Everything we eat contains sugar of some kind or is converted to sugar that can be used as fuel to run the body and the brain. When your blood is low on sugar, the first part of your body to notice the problem is your brain. Your brain uses 20-60% (depending on which medical expert you ask) of the sugar your body produces, so it makes sense that a low blood sugar level would affect your brain first.
When you eat, whether protein, carbohydrates, or straight sugar, your digestive system extracts the sugars and converts them to glycogen. The glycogen is then stored in your liver for use later. A type of sugar is also stored in your muscles and fatty tissues.
Insulin reacts to the level of circulating blood sugar, and it is insulin that "opens the cells" to release the sugar stored by the liver for use throughout your body. If you have a sugary snack (or an easily broken down simple carbohydrate like pasta), insulin is released to prevent your blood sugar from rising too much or to lower it if your sugar level has risen too high or too quickly. The idea is to keep your blood sugar level as constant as possible. A constant fuel supply is crucial to proper functioning of all of the body's systems, but this is especially true of the brain.
This is the way the system is supposed to work, but in hypoglycemia, it doesn't work properly.
If you have hypoglycemia, often the pancreas over-reacts and the level of sugar in the blood is pushed too low. At this point the adrenal gland sends adrenalin to signal the emergency condition and stops the release of insulin. The adrenalin also signals the liver and muscles to circulate some of the sugar that they have been storing to help bring the blood sugar back to normal operating levels. Additional hormones are produced to metabolize the sugar and block the insulin.
The release of adrenalin, also called epinephrine, causes the classic fight or flight response. This is where your initial symptoms of sweating, dizziness and elevated heart rate come from. Symptoms can be aggravated if your adrenal system is “burned out" from prolonged or frequent stress.
Other reactions vary widely, but you may find that you are hungry again soon after you eat, or you may crave sweets, or you may feel sick and dizzy. The symptoms range all the way from mild discomfort to the inability to function normally. In very severe cases, low blood sugar can cause passing out, coma, or even death.
Diagnosis & Treatment
Doctors are reluctant to diagnose anything without a corroborating test result, and that makes perfect sense. In the case of hypoglycemia, unfortunately, the test result is not conclusive because people can have the symptoms at blood glucose levels ranging from 80mg/dl down to 30mg/dl and below. (“Normal” is defined to be 100mg/dl.)
I can understand why doctors would like to set limits that define the line between "Yes, you have hypoglycemia" and "No, you don't have hypoglycemia." But it still seems strange to me that although we know that different people react differently to many things, it still seems to be so hard to understand that what one person's body interprets as a low blood sugar level is perfectly acceptable to another.
With the exceptions of fasting hypoglycemia, where there may be severe underlying conditions to treat, or drug or alcohol induced hypoglycemia where the cause must be found in order to avoid recurrence, hypoglycemia is always treated the same way—with diet.
Here are my top four eating rules for treating hypoglycemia:
• Eat 6-8 times per day. Don’t give your blood sugar a chance to drop!
• Eat protein with every meal and snack. Protein breaks down slowly and that keeps your blood sugar more stable.
• Choose high fiber foods. Whole grains and vegetables slow digestion and prevent your blood sugar from rising or dropping too quickly.
• Stop eating sugar. Sugar causes spikes in your blood sugar that result in sudden drops.
If you think you have hypoglycemia, see your doctor. If he/she confirms your hunch, try the four diet rules above. If he/she tells you there is nothing wrong with you, try the diet rules anyway. This is a healthy way for us all to eat. If you feel better on the diet, it really won’t matter whether you have hypoglycemia or not—feeling better is always a good thing!
